Per Cover Sheet, attach:
New Member Course Exam
Answer Sheet
Fingerprint cards (2)
Photo if requesting ID Card
Boating safety course completion certificate for requesting Basic
Qualification (BQ)
status if planning to become qualified in operations, or BQ may be
achieved later
Per ANSC 7001, attach:
U.S. Citizenship Verification
